Patient Rights & Responsibilities

As a PactRX Patient, You Have the Right To

  1. Be treated with dignity, respect, and consideration for your personal values and beliefs.
  2. Receive care without discrimination based on race, color, national origin, age, disability, sex, gender identity, sexual orientation, religion, or source of payment.
  3. Be informed about your medications, their intended purpose, potential side effects, and how to take them safely.
  4. Participate in decisions about your pharmacy care, including the right to refuse any medication or service.
  5. Have your personal health information protected in accordance with HIPAA and applicable laws.
  6. Receive a clear explanation of the cost of your medications before they are shipped.
  7. Speak with a licensed pharmacist at any time for medication counseling and clinical questions.
  8. Access your own medication records and request corrections to inaccurate information.
  9. File a complaint or grievance without fear of retaliation, and receive a timely response.
  10. Expect reasonable continuity of care if you choose to transfer to another pharmacy.
  11. Be informed of any financial relationship between PactRX and manufacturers, PBMs, or other entities that could influence your care.
  12. Receive services in a language you understand, including free interpretation and translation services as needed.

As a PactRX Patient, You Are Responsible For

  • Providing accurate information about your health, medications, allergies, and insurance.
  • Telling your Care Guide about any medications, supplements, or OTC products you take that weren't filled at PactRX.
  • Taking your medications as prescribed, and contacting us if you stop or change a medication.
  • Reporting adverse reactions, side effects, or concerns about your medications promptly.
  • Keeping your address and contact information up to date.
  • Being available to receive your shipments or making alternate arrangements.
  • Paying any copays or cash-pay charges owed for your medications.
  • Treating PactRX staff with respect.

A copy of these rights and responsibilities is provided at the start of service and is available any time on request.
